The route back into the system for anyone with no account: a beneficiary never paid, a card swallowed, a bribe demanded.
How to use this screen
- Choose the category that fits. It sets the clock your report runs on.
- Write a subject and describe what happened.
- Give a phone number if you want to be able to track the report — the last four digits are what the tracker checks.
- Tick anonymous if you would rather not be named.
- Submit. Write down the tracking code on the confirmation page.
Watch out
- Submissions are rate-limited per connection. If you have several reports to make, space them out or call the hotline.
- Keep the tracking code. Without it — and the phone number you gave — the report cannot be looked up again.
Good to know
- There is no puzzle to solve to submit this form. A person on a low-end phone should never be asked to prove they are human before reporting a stolen entitlement.
- If a field is missing, the form comes back with everything you typed still in it. A long account of what happened is never lost to a blank subject line.
- You can also call the FMHAPR hotline on 0800 CARE NGA.
